Accident Summary and Analysis
You are to prepare a 3-5 page report on an industrial accident. The accident must have occurred at an operation that is required to meet OSHA standards, and you are to take the perspective of someone who works for a different company in the same industry as the company at which the accident occurred. The report is for a supervisor who is concerned that the accident might be replicated at the operation at which you are employed, so the report needs to contain information about the events leading up to the accident, the accident itself, and the relevant OSHA standards. As part of your report comment on any ethical issues raised by the accident (refer to either the NSPE or SME code of ethics), and make sure that the report is both precise and concise.
